Job Description

The Licensed Psychiatric Mental Health Nurse Practitioner will work alongside the integrated mental health care team and primary care team providing psychiatric medication management and support.

  • Perform comprehensive psychiatric assessments, including history, mental status exam, and risk assessment
  • Diagnose mental health and substance use disorders in accordance with DSM-5 criteria
  • Initiate, prescribe, and manage psychotropic medications
  • Adjust treatment based on clinical response, side effects, and patient presentation
  • Monitor medication, adherence and effectiveness
  • Order and interpret laboratory tests as clinically indicated (e.g., metabolic monitoring, lithium levels)
  • Provide patient education on medication risks, benefits, and alternatives
  • Conduct follow-up visits focused on symptom management and medication response
  • Utilize standardized screening tools (e.g., PHQ-9, GAD-7) to guide treatment decisions
  • Document clinical outcomes and adjust care plans accordingly
  • Coordinate care with internal and external providers as needed
  • Facilitate referrals for specialty psychiatric or higher-level care
  • Support transitions of care and follow-up after hospitalization
  • Manage a panel of patients requiring psychiatric medication services
  • Support timely access to psychiatric care, including same-day or urgent needs when appropriate
  • Contribute to improved clinical outcomes and access metrics

Supervisory ResponsibilitiesThis position does not directly supervise anyone.

Work EnvironmentThis job operates in a face-paced primary care setting. This role may occasionally come into contact with patients who may have contagious illnesses.

Physical Demands
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift 15 pounds at times.

Travel RequirementsMust have the ability to travel between MCC sites in the St. Paul/Minneapolis metro area.

Required Education and Experience
  • Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program
  • Completion of an accredited Psychiatric Mental Health Nurse Practitioner (PMHNP) program
  • Current, unrestricted Registered Nurse (RN) license in the State of Minnesota
  • Current, unrestricted Advanced Practice Registered Nurse (APRN) license in the State of Minnesota
  • Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) through the American Nurses Credentialing Center
  • Current and active DEA
  • Current Basic Life Support or CPR certification
  • 1-2 years' experience in Psychiatric care preferable in a Federally Qualified Health Care (FQHC) or community setting.

Additional Eligibility Requirements
  • Demonstrated success in working effectively with target population(s).
  • Change Agile; ability to operate in the gray and flex to new developments or situations.
  • Experience working in a multi-site environment is highly desired.
  • Fluent in second language preferred (Spanish and or Hmong)
